Various metabolic disorders that contribute to stone formation in the urinary organs lead to changes in urine pH either to the acidic or alkaline side. Acid-base homeostasis of urine largely depends on the exchange of urinary citrate and ammonium.
Changes in urine pH are important for the processes of crystallization of uric acid and phosphates. A urine pH value of 6.2-6.4 is considered normal, which ensures a stable colloidal state of urine. Normally, the kidneys reabsorb almost all of the filtered bicarbonate (approximately 85% of the filtered bicarbonate is reabsorbed in the proximal tubule, the remaining 15% of the filtered bicarbonate is reabsorbed in the distal tubule), but this does not make the urine acidic. Acidification of urine occurs due to the secretion of hydrogen ions in the distal tubules (which is partly dependent on mineralocorticoids) and the secretion of ammonium ions (this produces ammonium ion in acidic urine). With a slightly acidic or neutral urine reaction, uric acid, even at high concentrations, does not precipitate and is in an ionized (dissolved) state. When the pH of urine decreases, the solubility of uric acid drops sharply, since most of it passes into a non-ionized form, which is poorly soluble and easily precipitates into a crystalline precipitate.
A decrease in the level of citric acid in the urine leads to an increase in the crystallization of calcium oxalate, since this acid is known to act as an inhibitor of the crystallization of calcium salts.
Hypocitraturia, as a causative factor of urolithiasis, is also associated with the exchange of hydrogen, bicarbonate and ammonium ions. A decrease in citric acid concentration may be idiopathic or a secondary condition (as a consequence of metabolic acidosis, decreased potassium levels, thiazide diuretics, decreased magnesium concentrations, renal tubular acidosis, diarrhea). Normally, citric acid is freely filtered by the glomeruli of the kidneys and 75% of it is reabsorbed in the proximal convoluted tubules. Most secondary causes also lead to a decrease in urinary citrate excretion due to increased reabsorption in the proximal tubules. In humans, approximately 10–35% of filtered citrate is excreted in the urine, so citrate is the main organic anion in urine. Reabsorbed citrate is completely metabolized to CO2 and H20, which provides up to 10% oxidation of urine. Thus, urinary citrate is citrate that has been filtered but not reabsorbed. Therefore, the amount of urinary citrate is predominantly determined by reabsorption and, to a lesser extent, by filtration, because Plasma citrate concentration is low. Ingested citrate is metabolized to bicarbonate and is therefore used as a urine alkalinizer and increases urinary citrate levels.
Impaired acid-forming function of the kidneys, hypocitraturia and high urine density are the most common disorders that contribute to stone formation in the urinary organs. In our study, 1,182 patients with urolithiasis were examined.
Violations of urine pH in patients with urolithiasis were detected in 941 patients, which is 79.6%. Including a decrease in pH less than 6.0 - 896 people (75.8% of the total number of patients examined). An increase in pH (more than 6.5) was noted in 45 patients, which amounted to 3.8% of all patients. Uric acid diathesis, in which uric acid is in a state of supersaturation, even at normal concentrations, was detected in 529 (44.7%) cases. Moreover, as a mono-violation - in 181 (15.3%) cases. High urine density, which may indirectly indicate low urine volume, was detected in 969 (82%) cases.
Hypocitraturia was detected in 253 patients, which was 21.5% of all patients examined. In two cases (0.2%), the cause of hypocitraturia was renal tubular acidosis. In other cases, it is not possible to establish the cause of hypocitraturia and we regarded the condition as idiopathic hypocitraturia (251-21.3%). As a mono-disorder, hypocitraturia was identified in our study in 58 cases, which amounted to 5.0%. A significant difference in the concentration of citric acid in urine was also revealed between healthy people in the groups of calcium stones in general (P<0.05), as well as calcium oxalate stones (P<0.1) and calcium stones with a high level of uric acid in the urine ( P<0.05). Among other groups, differences occurred in xanthine (P<0.1) and 2,8-dihydroxyadenine lithiase (P<0.05). When comparing the results of the study and taking into account the different prevalence of these types of stone formation, it should be noted that hypocitraturia is a pathogenetic factor mainly in the group of calcium stones.
Therapeutic measures aimed at relieving disorders of acid-forming kidney function, hypocitraturia and high urine density are not considered as an alternative to stone removal or specific therapy for metabolic disorders. Rather, this treatment is nonspecific and can be recommended for most patients with urolithiasis.
